During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/8/25 from 10:45 am to 11:45 am. The inspector reviewed compliance in the areas listed above. There were 4 daycare children and 1 resident child present and 2 caregivers; the point total was 11. The inspector reviewed 2 children?s records and 2 caregiver's records on-site. This inspection included document review tour of the family day home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-140-B-1
A substitute provider must have (i) A high school program completion or the equivalent or (ii) evidence of having met the requirements for admission to an accredited college or university.
The provider stated over the phone that Caregiver # 2 was the substitute provider in her absence. Caregiver # 2 did not have documentation of a high school diploma or anything equivalent to that, on file.
Plan of Correction: We will ensure that proper educational documentation is on file for substitute provider.
Standard 8VAC20-800-150-C
An assistant 18 years of age or older shall not be left alone with children in care for more than two hours per day.
The attendance record showed that on 7/7/25, Caregiver # 1 who is an assistant was by herself with one child for 2 hours and 30 minutes.
Plan of Correction: Caregiver # 1 will not be left alone with children over two hours.
Standard 8VAC20-800-150-D-1
An assistant supervising children alone must have Current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care.
Caregiver # 1 was alone with children in care after 3 pm on 7/3/25 and 7/7/25. Caregiver # 1's school transcript listed her CPR training but there was no documentation of the date of completion and if it was intended for the age group in care.
Plan of Correction: Certification will be obtained for Caregiver # 1 to meet the proper requirement.
Standard 8VAC20-800-150-D-2

Safety straps were not in use while a high chair was in use by a child.
Child # 1 was observed seated in a high chair. Child # 1 was not buckled using the safety straps.
Plan of Correction: Children will be buckled while using high chairs and other equipment that require safety straps.
