Violations

1
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: The wall at the back left corner of the preschool classroom near the double door closet had a hole approximately 1.5 inches in diameter near the right door of the closet and insulation in the wall was exposed. There were no children observed at the closet but the area is accessible to children.
Plan of Correction: Provider will have hole in wall patched.
