Inspector Notes
An unannounced monitoring inspection was conducted on 8/10/2023. There were 63 children, ages 8 months -12 years, 18 staff and 3 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed. A total of 5 children?s records and 5 staff records were reviewed. The children were observed during center time, lunch and naptime. The inspector arrived for the inspection at 10:17am and departed the center at 1:18pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
5Evidence: Child 3 start date was 8/7/2023 and on the day of the inspection did not have the physical examination signed by a physician.
Evidence: The annual fire inspection was dated 6/14/2022 and on the day of the inspection a current annual fire inspection was not documented.
Evidence: The counter with the sink in the two?s classroom had a bottle of Dawn dish detergent sitting on the left side of the counter at the front edge. No children were observed in the area but it was accessible to children.
Evidence: Licensing staff observed approximately 4 to 6 inches of resilient surfacing around the fall zones of the 4 foot tall green triangle shaped climbing structure.
Evidence: The changing pad in the two?s classroom had a tear in the material approximately 1/4 inch long at the right, front corner and the padding was exposed.
