Inspector Notes

An unannounced, on-site renewal inspection was initiated on 2/24/2025 and completed on 2/24/2025, as a part of the licensure period. The on-site inspection began at 10:07AM and ended at 11:48AM. The inspector reviewed compliance in the areas listed above. There were 69 children present and 14 staff. The inspector reviewed 3 children?s records on site on 2/24/2025 and 3 staff records on 2/24/2025. Medication was reviewed on site 2/24/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunization records and physical records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record and physical record for child #1 was not signed.
Plan of Correction: Provider will obtain signed physical and immunization record.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. Three tricycles were observed on the playground with holes in the back of the seats with sharp edges. Children were in the area and using the equipment.
Plan of Correction: Provider will replace the seats.
