The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 2/27/2025, were not posted in a conspicuous place on the premises.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children that is signed by a physician. Staff #1, employed for 1 month, TB screening was not signed by a physician.

The center shall provide an annual fire inspection. The annual fire inspection was overdue by 3 weeks.

REPEAT VIOLATION
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. Two tricycles were observed on the playground with holes in the back of the seats with sharp edges.

Where playground equipment is provided, resilient surfacing shall comply with minimum
safety standards. The resilient surfacing around the fall zones of both playground structures measured 1 inch.

Nonprescription medication shall be given to a child only with written authorization from the parent. Medication for Child 2, age 7 months, was administered on 11/4/25 and 11/5/25 without written authorization from the parent.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. On 11/6/2025, there were 14 children present in the Pre K class, but only 12 were marked present on the attendance record.
