Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on 8/26/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 62 children present, ranging in ages from 8 months to 5 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Resilient surfacing on the playground was observed and in compliance. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-160-A
Based on record review and staff interview, the center did not obtain documentation of a negative tuberculosis (TB) screening for all staff within 21 days after employment.

Evidence: Staff #2 (Date of Hire: 5/3/2021) did not have TB screening on file.
Plan of Correction: The two staff below were the only two staff needing current TB test. Staff #2 is currently getting her TB test.

Based on record review and staff interview, the center did not ensure that all staff submit the results of a follow-up tuberculosis screening at least every two years.

Evidence: Staff #4's most recent TB test on file is dated 5/4/2019.
Plan of Correction: Staff #2 and 4 were the only two staff needing a TB test. Staff #4 got her TB test on 8/27/2021.
