Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was conducted on 8/26/2021. There were 62 children present with 13 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not ensure that all staff who work directly with children obtained first aid training certification within 90 days after employment.

Evidence: Staff #2 (Date of Hire: 5/3/2021) does not have documentation of completing first aid training on file.
Plan of Correction: Staff #2 is completing her first aid training.

Based on record review, the center did not ensure that all staff who work directly with children have completed the Preservice Update training.

Evidence: Staff #4 completed Preservice training on 3/9/2017 but has not completed Preservice Update training.
Plan of Correction: Staff #4 will complete her 3 hour training by the end of the week.
