Inspector Notes

An unannounced complaint investigation was conducted beginning on 10/18/2022 and concluding on 11/30/2022 and was conducted jointly with local law enforcement and Child Protective Services. The Northern Licensing Office received the complaint on 10/17/2022 regarding forbidden actions. Interviews were conducted, records and photos reviewed, and observations made in the center. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid. The violation cited was found during the course of the investigation.

Based on interviews, review of documentation and photos, the center did not maintain a written record of children's serious and minor injuries in which entries are made on the day of occurrence.

Evidence: Child #1 (Age 2) sustained multiple injuries including bruising and scratches on Thursday 10/13/2022 and the child's record contained no documentation of the injuries. Staff #1 and Staff #2 both noticed marks on Child #1 on 10/13/2022 but did not write an injury report.
