Inspector Notes
An unannounced monitoring inspection was conducted 3/202/2023 from 10:15am-1:35pm. There were 80 children directly supervised by14 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 6 staff records, 8 children?s records, 5 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, engaging in age appropriate free play, and eating lunch which consisted of chicken pilaf, green beans, peaches, and milk. Infant care including diaper changing and handwashing was also observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

6Evidence: Staff #4 (Date of Hire: 11/4/2022) did not have the results of a Virginia Child Abuse and Neglect Central Registry search on file.
Evidence: Staff #2 (Date of Hire: 12/16/2022) has not yet completed the Virginia Department of Education-sponsored orientation course.
Evidence: In the Twaddler classroom, an unlocked spray bottle of bleach was observed sitting on the counter in the diapering area. There were no children being diapered at that time.
Evidence: There was a bottle of prepared infant formula in the refrigerator in the Infant A classroom for Child #1 that was not dated.
Evidence: Staff #2 (Date of Hire: 12/16/2022) did not have reference checks on file.
Evidence:
1. In the Infant A room, Child #1 was present but was not signed in on the classroom attendance sheet.
2. In the Infant B room, there were 4 children present and only 3 signed in on the classroom attendance sheet.
3. In the Twaddler room, there were 14 children present and only 13 signed in on the classroom attendance sheet.
4. In the Prepper room, there were 8 children present and 11 were signed in on the classroom attendance sheet.
5. In the Toddler A room, there were 5 children present and 7 were signed in on the classroom attendance sheet.
6. In the 3's and 4's room, there were 10 children present and 11 were signed in on the classroom attendance sheet.
