During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/5/2025. The on-site inspection began at 10:30am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 73 children present and 13 staff. The inspector reviewed 7 children?s records and 5 staff records on-site.
This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/24/2025. A POC submitted after this date will not appear on the public website.
Violations
9There was no documentation of immunizations in the record for Child A, who had been enrolled for over 3 months.
There was no physical exam in the record for Child A, who had been enrolled for over 3 months.
1. In the toddler A classroom, there was a spray bottle with bleach/water solution unlocked in an upper cabinet.
2. In the infant B classroom, there was a spray bottle with bleach/water solution unlocked in an upper cabinet.
The medication policy for the center states expired medication will not be accepted, the medication for Child B, expired over 4 months ago.
The record for Child A, enrolled for over 3 months, was missing proof of the child's identity and age.
Lockdown drill was not documented for the year 2024.
In the Preschool 2B classroom, there was one lunch bag with no name and date, there were several snacks in a cubby with no name and date.
Staff A, who has been actively working at the center for the past 3 months, had only one reference checked prior to employment.
1. In the twaddler classroom, there were 7 children present, but 8 children were signed in the daily attendance.
2. In the infant A classroom, there were 3 children present, but 2 children were signed in the daily attendance.
