During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/2/2025. The on-site inspection began at 1:00pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 7 staff. The inspector reviewed 7 children?s records and 4 staff records on-site.
This inspection included document review, a tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/10/2025. A POC submitted after this date will not appear on the public website.

5In the Infant B classroom, there was a bleach and water spray solution in an unlocked cabinet in the front of the classroom as well as a spray and bleach spray solution in an unlocked cabinet in the rear of the classroom.
1. The record for Child #1 was overdue by 3 years for a parent?s signature for an annual review of the child's record.
2. The record for Child #2 was overdue by 2 years for a parent?s signature for an annual review of the child's record.
3. The record for Child #3 was overdue by 1 year 3 months for a parent?s signature for an annual review of the child's record.
4. The record for Child #10 was overdue by 1 year 11 months for a parent?s signature for an annual review of the child's record.
5. The record for Child #11 was overdue by 1 year 10 months for a parent?s signature for an annual review of the child's record.
There was no documentation on file for an evacuation drill for the month of August 2025.
1. In the Infant B classroom, Child #5 had food brought from home that was not dated.
2. In the Toddler B classroom, Child #6 & #7 had food brought from home that was not dated.
3. In the Twaddler classroom, Child #8 had food brought from home that was not dated.
4. In the School-Age classroom, Child #9 had food brought from home that was not dated.
In the Infant A classroom, Child #4 had 3 bottles in the refrigerator with no dates.
