When staff are supervising children, they shall always ensure their care, protection, and guidance.
On two separate occasions Staff A and B did not ensure proper care and guidance:
(COMPLAINT RELATED) 1. On 1/28/2026, in the Preppers Classroom, Staff A was trying to get Child A (Age 2) from behind the door, Staff A roughly handled Child A to get child from behind the door.
2. On 2/10/2026 during the inspection, live video footage from the Preschool Classroom showed Staff B attempting to keep Child B on their cot during rest time. Staff B was observed pulling the child roughly by the leg in order to return the child to their cot. When Child B (Age 4) attempted to turn around, Staff B used force to push the child by the shoulder to reposition the child on the cot.

On three separate occasions Staff A and B did not use behavior guidance constructive in nature intended to redirect the children to appropriate behaviors:
(COMPLAINT RELATED) 1. On 1/28/2026, in the Preppers Classroom, Staff A was trying to get Child A (Age 2) from behind the door, Staff A roughly handled Child A to get child from behind the door.
(COMPLAINT RELATED) 2. On 1/28/2026 in the Preppers classroom, Staff A was observed telling the children that due to their behavior, the staff member was leaving. Staff A then put on a coat and pretended to exit the classroom. During this time, multiple children were observed screaming and crying in response to Staff A?s actions.
