An anonymous complaint was received in our office of May 26 2023 that alleges an issue with the physical plant. A focused inspection was conducted on the physical plant on June 14, 2023 between 8:30am and 9:00am. The allegation was deemed valid. Based on direct observation, the center did not ensure that areas and equipment of the center shall be maintained in a clean, safe, and operable condition.
Evidence: A fence slat was not properly secured to the fence and was able to be moved with minimal force. A nail was able to be seen protruding from the back of the fence slat.

Plan of Correction: 6/14/2023 - Children are not outside because of weather. Called maintenance person. Children will not be allowed outside until fixed. We will notify [inspector] when repaired and we will continue inspecting our playground daily before children play outside.
