Inspector Notes

An unannounced, on-site renewal inspection was completed on 09/12/2024, as a part of the licensure period. The on-site inspection began at 9:50 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 73 children present and 17 staff. The inspector reviewed 5 children?s records and 5 staff records on site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/19/2024. A POC submitted after this date will not appear on the public website.

Based on record review, the annual fire inspection was outdated.

Evidence:
The last fire inspection was older than a year as it was dated 08/18/2023.
Plan of Correction: The fire inspection is scheduled for the week of 9/16/2024.

Based on record review, the center did not obtain annual approval from the health department.

The last health inspection was completed on 02/04/2023.

Based on observation, areas and equipment of the center were not maintained in a clean, safe, and operable condition.

Evidence:
In the First Steps classroom, there was a hole in the wall near an outlet, within reach of small children.
Plan of Correction: The repair of the hole in the First Steps classroom has been scheduled for 9/19/2024.

Evidence:
Two bottles in the infant room did not have the date to be served on them.
