Inspector Notes
An unannounced monitoring inspection was completed on 11/5/2025 in response to a self-reported incident related to feeding protocol, and two injury-related incidents, all received by the licensing office on the following dates: 09/22/2025, 10/16/2025, and 10/31/2025. The inspector reviewed 3 children?s records and 7 staff records 11/05/2025. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
