Inspector Notes

A monitoring inspection was conducted on 6/21/2022 with the center director. There were 26 children present, ranging in ages from 17 months to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 0 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the facility failed to maintain all areas and equipment of the center, inside and outside, in a safe and operable decision. Evidence: in the infant and toddler room, there was a light fixture missing with exposed wires hanging from the ceiling. In the preschool and school age room, the soffit was cracked and coming apart from the ceiling. Outside, the wooden fence around the air conditioning heat pump was broken, leaving the unit accessible to children.

Based on observation, the facility failed to maintain the required ratio of one staff for every 5 children ages 16 months up to 24 months. Evidence: Staff A was alone with 12 children ages 17 months to 12 years during the licensing inspection from 3:00 p.m. to 5:00 p.m.

Based on observation, the facility failed to maintain the ratio of 1 staff person for every 10 children ages 3 years old up to school age eligibility. On 6/21/2022, Staff B was alone with 15 children ages 3 years old to 9 years old.
