During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/7/2024 and completed on 10/7/2024. The on-site inspection began at 9:40am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/28/2024. A POC submitted after this date will not appear on the public website.

The center is required to have employees undergo a background check every five years.

The most recent fingerprint background check for staff #4 was overdue by over 9 months.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The immunization record for child #2 was received over 30 days after the child?s first date of attendance.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #5, who had been attending the center for over 5 months. The physical examinations for child #1 and child #2 were received over 30 days their first day of attendance.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

There was an upper cabinet with a broken, unsecured cabinet door observed during the inspection posing a safety hazard to children.

Staff #2, Staff #3 and staff #5 were observed supervising 18 children (youngest child 4 months) in one room from approximately 9:40am to 9:55am.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for child #1, enrolled for over 4 months, was missing the following required components: parent home address, parent phone number and parent work phone number. The record for child #2, enrolled for over 4 months, was missing the following required components: parent phone number, parent place of employment, and parent work phone number. The record for child #3, enrolled for over 7 months, was missing the following required components: the name, address and phone number for 1 parent, work phone number and addresses for two designated people to call in an emergency. The record for child #5, enrolled for over 5 months, was missing the following required components: home address, work phone number and name/address/phone number of 1 parent.
