Inspector Notes
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/04/2021 and concluded on 05/10/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present with 2 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 caregiver records and 2 children's records submitted by the provider to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Violations
31. Child 1, enrollment date 08/11/2019, lacks documentation of a current physical exam.
2. The provider confirmed that documentation of a current physical exam was not on file for child 1.
Evidence:
1. Six children were observed napping on rest mats without a top cover or bottom cover.
2. The provider did show the inspector her supply of sheets and blankets stored on shelves in the laundry room.
Evidance:
1. The most recent immunizations, for child 1, were administered on 09/26/2019, according to the written immunization record on file in the family day home.
2. The provider confirmed that an updated immunization record was not on file.
