Inspector Notes

This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/04/2021 and concluded on 05/10/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present with 2 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 caregiver records and 2 children's records submitted by the provider to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Standard 22VAC40-111-100-A
Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

1. Child 1, enrollment date 08/11/2019, lacks documentation of a current physical exam.
2. The provider confirmed that documentation of a current physical exam was not on file for child 1.
Plan of Correction: The parent of child 1 will be asked today to provide a current physical.
Standard 22VAC40-111-530-A
Based on observation and interview, the provider failed to ensure that rest mats when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.

Evidence:
1. Six children were observed napping on rest mats without a top cover or bottom cover.
2. The provider did show the inspector her supply of sheets and blankets stored on shelves in the laundry room.
Plan of Correction: The provider stated that the children complained of being hot and were therefore not given a sheet and a blanket. Going forward, all children will be offered linens at nap.
Standard 22VAC40-111-90-C
Based on record review and interview, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidance:
1. The most recent immunizations, for child 1, were administered on 09/26/2019, according to the written immunization record on file in the family day home.
2. The provider confirmed that an updated immunization record was not on file.
Plan of Correction: The parent of child 1 will be asked to provide an updated immunization record.
