Inspector Notes
An unannounced monitoring inspection was conducted on 12/08/2022 from 10:30 am - 12:15 pm. There were 9 children in care for a total of 21 points. Two caregivers were present during the inspection. Children were observed during group story time. A visit from Santa to include a gift exchange was observed.
Records reviewed for seven children and five caregivers/household members.
Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the family day home.
Violations
5Evidence:
1. The employment file for caregiver 1 lacked the following required documentation:
a. Hire date;
b. Name, address and telephone number of a person to be notified in an emergency;
c. Two or more references as to character and reputation as well as competency to be checked before employment.
Evidence:
1. A TB screening was not on file for household member 1.
a. The provider verified that documentation of a TB screening was not available for household member 1.
Evidence:
1. There was not written documentation on file to demonstrate that caregiver 1 had received orientation training by the end of their first week of employment.
Evidence:
1. The attendance sheet for the family day home could not be located by either caregiver 1 or caregiver 2.
2. Both caregivers indicated the attendance sheet had been completed however, neither caregiver could find the attendance record during the inspection.
Evidence:
1. When the inspector arrived at 10:30 am caregiver 1 was observed alone with a total of 9 children whose ages counted for a total of 21 points.
a. There were no additional caregivers in the child care room with caregiver 1.
2. Caregiver 2 arrived right after the inspector arrived and both the inspector and caregiver 2 entered the family day home together.
