A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/16/2025. A POC submitted after this date will not appear on the public website.
Violations
4A food allergy/sensitivity listed on the enrollment form for child 1 was not included on the food allergy list maintained by the provider.
3 of 5 children in care were not signed in on the attendance sheet when the inspector arrived.
The last day of employment was not documented in the file of caregiver 1.
Caregiver 2 has not completed the department's health and safety update course since 2023.
