Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/16/2025. A POC submitted after this date will not appear on the public website.

The vendor shall maintain, in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions.
A food allergy/sensitivity listed on the enrollment form for child 1 was not included on the food allergy list maintained by the provider.
Plan of Correction: The child who's name was not on the food allergy form has no allergy listed by a doctor. The mother states he don't like milk.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
3 of 5 children in care were not signed in on the attendance sheet when the inspector arrived.

Caregiver records shall include the date of separation from employment where applicable.
