Inspector Notes

A monitoring inspection was conducted on 5/12/2022. There were 33 children in care, with a total of 8 staff members assigned to their direct supervision. The Inspector reviewed compliance in the areas of administrative records, background checks, emergency preparedness, nutrition, and staff training.

Based on a review of staff background checks it was determined that one staff person has not submitted to a Fingerprinting criminal history background search.
Evidence:
Staff B has not completed a Fingerprint national criminal history background search.
Plan of Correction: The fingerprinting appointment was made today.
Standard 8VAC20-770-60-B
Based on a review of staff background checks, it was determined that one staff person was employed prior to completing a Sworn Disclosure Statement.
Evidence:
Staff B has been employed without signing a Sworn Disclosure Statement.
Plan of Correction: The statement form was signed immediately today.
Standard 8VAC20-770-60-C-2
Based on a review of staff background checks, it was determined that one staff person has been employed more than 30 days without a complete Virginia Central Registry background search.
Evidence:
A search of the Virginia Central Registry has not been completed for Staff B.
Plan of Correction: The form was notarized today and will be submitted today.
Standard 8VAC20-780-60-A
Based on a review of children's records it was determine that one record does not include all required information.
Evidence:
Child Record B does not include documentation indicating that a staff person has viewed the child's original proof of identification.
Plan of Correction: The parent will be asked to bring the birth certificate tomorrow.
