Inspector Notes

A renewal inspection was conducted on February 6, 2023 between the hours of 1:10 pm and 3:10 pm. There were 40 children present, with 6 staff supervising within the supervision guidelines. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on record review, the center failed to obtain documentation that each child has received the immunizations as required before the child can attend the center.
Evidence: Child #4 has a start date of 1/3/23 and there is no documentation of immunization on file.
Plan of Correction: I will contact the parents and tell them I need them today.
Standard 8VAC20-780-140-A
Based on record review, the center did not obtain for each child a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence: Child #4 (start date 1/3/23) did not have documentation of a physical examination on file at the time of inspection (2/6/23).
Plan of Correction: I will contact the parents and tell them I need the physical today.
Standard 8VAC20-780-330-B
Based on observation, the playground did not have resilient surfacing to meet the minimum safety standards for play areas.
Evidence: Mulch is barely covering the dirt under and around the swing/slide and climbing equipment.
Plan of Correction: I will get this corrected within the next 4 weeks.
Standard 8VAC20-780-350-B-4
Based on observation, the center did not ensure the staff-to-children ratio as specified was maintained when children are in care.
Evidence: The Fireflies class (ages 4-6) had eleven (11) children with one (1) teacher supervising.
Plan of Correction: I will ensure our teacher-to-child ratios are maintained.
Violation corrected during the inspection.
Standard 8VAC20-780-80-A
Based on observation, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The Fireflies (age 4-6) class did not have daily attendance documented for the arrival of children in care at the time of inspection.
2. The Dragon Flies class (age 4-5) did not have daily attendance documented for the arrival of each child in care.
Plan of Correction: We will maintain a written daily attendance for the arrival and departure of each child.
