During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on January 22, 2025. The on-site inspection began at 11:45am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
All employees shall undergo a criminal history background check every five years from the date of the first search.

The criminal history background check for staff #1 has not undergone a criminal history background check every five years, search is overdue by 21 months.
Plan of Correction: Staff will make an appointment and get the fingerprint done. Results will be sent to the inspector when received.
Standard 22.1-289.036-A
An agent who acts on behave of the business shall submit to a criminal history search and search of the central registry every five years.
Agent #1 has not submitted to a criminal history search or central registry search every five years. Current search's expired over 4 years ago.
Plan of Correction: I will ensure the owner completes all the required background checks for this location. I will send the inspector the results when received.
Standard 8VAC20-780-40-I
The center shall develop written procedures for injury prevention.

The center has not developed a written procedure for injury prevention.
Plan of Correction: I will make sure an injury prevention plan is completed annually.
Standard 8VAC20-780-550-O
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence and contain all the required information.

6/13 written records of children's injuries contained the required information. Missing information included two signatures, actions to prevent future occurrence and child's names were not complete.
Plan of Correction: I will make sure all accident reports are filled out completely.
