Inspector Notes

A renewal inspection was initiated on 9/15/22 and concluded on 9/15/22. There were 38 children present, ranging in ages from 5 months to 4 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-530-A
Based on record review, interview and observation, the center failed to ensure at least one staff in each classroom was present with CPR and first aid certification.

Evidence:

1. The director verified that staff 2 is the lead teacher and supervises the 3 year old classroom alone. The record for staff 2 did not contain documentation of staff 2 completing CPR and first aid certification.
2. The director stated staff 2 had not completed CPR and first aid certification yet.
Plan of Correction: Staff 2 will be scheduled for a CPR and first aid class as soon at it can be scheduled.

Based on record review and interview, the center failed to ensure all required information was obtained for staff records.

Evidence:

1. The records for staff 2, 3, 4 and 5 did not contain the name address and phone number of an emergency contact.
2. The director verified that the emergency contact information for not obtained for the four staff.
Plan of Correction: In the future the emergency contact information will be obtained and documented upon hire.
