Inspector Notes

A monitoring inspection was initiated on 12/13/2023 and concluded on 12/13/2023. There were 48 children present, ranging in ages from 4 months to 5 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 6 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to obtain central registry search results every five years for all employees.

Evidence:
1. The record of Staff 1 contained documentation of a central registry dated 06/08/2018, which expired 06/08/2023. Documentation of an updated central registry check dated 06/14/2023.
Plan of Correction: I will make sure that the background checks are completed before the 5 years have expired.
