Inspector Notes

A monitoring inspection was initiated on 7//29/2022 at 10:05am and concluded on 7/29/2022 at 11:05am. There were a total of 5 children (13 points) observed with 2 caregivers present. The children were engaged in the following: toy play, tummy time, clean up time, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 5 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, the central registry check and Sworn Statement for the provider was expired.

Evidence: Central Registry Check for Staff #1 expired 3/10/2022 and Sworn Statement expired 2/23/2022.
Plan of Correction: Provider will obtain and retain a current Central Registry Report and complete and retain a new Sworn Statement.
Standard 8VAC20-800-180-A
Based on file review, 2 adult household members did not have a current report of tuberculosis screening form every 2 years from the date of the first screening.

Evidence: Household member #1 had a tuberculosis screening that expired 7/24/2020, and household member #2 had a tuberculosis screening that expired 10/4/2021.
Plan of Correction: Provider will obtain and retain a new Tuberculosis screening for both household members.
