Inspector Notes

The licensing inspector conducted a scheduled modification inspection on Monday, December 19, 2022. The provider is requesting to modify the license due to a change in location. The request to modify the license was received 12/12/2022. No children were in care. The provider's home had been under construction and the family day home had relocated during construction. Now that the construction is completed, the family day home will return to it's original location.The provider will utilize the living room, kitchen and the fenced in backyard for child care purposes.
