The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a central registry check or a search of the sex offender registry for one state for staff 1, who indicated on her most recent sworn statement that she has resided in another state within the past 5 years.
Plan of Correction: The center responded with the following: Staff 1 moved to another state during her employment and the out-of-state background checks were mistakenly overlooked. The background checks were completed during the inspection and have been placed in staff 1's record.

The center shall implement a monthly practice evacuation drill. The center did not conduct a practice evacuation drill for the month of November 2025.
