Inspector Notes
A monitoring inspection was conducted on 4/19/22. There were 13 children present (35 points) with 3 caregivers. One of the 13 children present was a resident child. The children were observed playing with toys, engaging in music and movement activities and eating snack. Children were observed washing hands before eating snack and after using the bathroom. Currently, the children are bringing food from home. A total of 3 children's records, 3 caregiver's records and 1 pet's record was reviewed. There were no medications on site.

Violations
2Evidence:
1. Caregiver # 1, Caregiver # 2 and Caregiver # 3's central registry findings expired on 2/15/22.
2. Caregiver # 1 stated that she had submitted new forms in December 2021, but there was no documentation available to support this information or to show that any follow up was completed when the results were not received within 30 days of mailing the forms.
Evidence:
1. The inspection started at 9:35 am, at which time Caregiver # 1 and Caregiver # 2 were present with 13 children. Out of the 13 children present, 1 child was a resident child (3.5 years old).
2. Caregiver # 1 stated that Caregiver # 3 had left to run an errand and would be back shortly. Caregiver # 3 did not return until 10:20 am.
3. Both caregivers had a total of 35 points, which exceeded the maximum of 32 points allowed for 2 caregivers.
4. The provider was over by 3 points for a duration of approximately 45 minutes.
