Inspector Notes

An unannounced monitoring inspection was conducted on 4/25/23 from 11:20 am to 12:20pm. There were 12 daycare children and 1 resident child (29 points) present, with 2 caregivers supervising. The children were observed eating lunch that was brought from home, playing with toys and washing hands. 3 children's records, 3 caregiver's records, emergency drill records, attendance and pet vaccination records were reviewed. There were no children with medications on site.

Standard 8VAC20-800-240-A
Based on observation, an area inside the family day home was not maintained in a safe condition.
Evidence: The vinyl flooring by the entrance door was broken in three places. One of the tears left jagged edges posing an injury hazard to the children.
Plan of Correction: Floor will be replaced by next weekend. A carpet has been placed to avoid injuries.
Standard 8VAC20-800-750-A
Based on observation, topical ointments were not labeled with the child's name
Evidence: One skin moisturizing tube and one diaper rash cream was not labeled with the children's names.
Plan of Correction: Placed name and expiration date and assured all were labeled.
Standard 8VAC20-800-750-D
Based on observation, a product past it's expiration date was in use.
Evidence: One child's skin moisturizing cream expired in 11/2021.
Plan of Correction: Threw product away.
Standard 8VAC20-800-800-B
Based on documents reviewed, the provider did not review the emergency plan at least annually.
Evidence: The emergency plan was last updated on 1/6/22.
Plan of Correction: The plan was updated on 1/4/23. It was missed.
