Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 4/23/25 from 11:00 am to 11:45 am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 caregivers; the point total was 29. The inspector reviewed 2 children?s records, 2 caregiver records, and 1 household member's record on-site. This inspection included document review, tour of the family day home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each adult household member a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening.
Household member # 1's TB screening was completed in 2023. A current TB screening was not obtained in two years.
Plan of Correction: The TB screening will be scheduled.
