During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 16, 2025. The on-site inspection began at 12:20pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 23 children present and 4 staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 11/04/2024, were not posted in a conspicuous place on the premises.
Plan of Correction: I will separate the inspection report, so parents can have access the entire inspection. I will ensure the complete inspection report is posted going forward.

The violation was corrected during the inspection.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A bottle of disinfectant with warning label, ?Keep out of reach of children,? was in the unlocked changing table cabinet in the boys restroom, within reach of children.
Plan of Correction: The staff must have forgotten to return the disinfectant to the locked cabinet. I will remind all staff this must be kept in a locked cabinet.

The violation was corrected during the inspection.
Standard 8VAC20-780-520-C
If diaper ointment or cream is used, the product shall be labeled with the child's name. A container of A&D Prevent diaper ointment in the boys restroom that was not labeled with a child's name.
Plan of Correction: The name will be reapplied to the container.
