During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 10, 2025. The on-site inspection began at 11:30am and ended at 1:05pm. The inspector reviewed compliance in the areas listed above. There were 14 children present and four staff. The inspector reviewed four children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

The fall zones around the playground equipment had less than 3 inches of mulch. When mulch is used as a resilient surface in fall zones there must be 6 inches of mulch underneath and surrounding equipment up to 6 feet on all sides.
Plan of Correction: Notified maintenance today and the mulch will be added by 9/21/25.
Standard 8VAC20-780-450-A
Cots or mats used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Classroom #1 did not have a top and bottom cover for mats used by children during the designated rest period.
Plan of Correction: I will notify the parents today that children need to bring a top & bottom cover for rest time by 9/11/25.
