Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 19, 2026 as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 21 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member shall submit documentation of a negative tuberculosis(TB) screening at the time of employment and within the last 30 calendar days of the date of employment.

Staff #1, employed for two months, had documentation of a tuberculosis screening dated five months before the first date of employment. Staff # 5, employed for five months, had documentation of a tuberculosis screening dated eleven months before the first date of employment.
Plan of Correction: Moving forward, the center will ensure that staff will submit a negative tuberculosis screening at the time of employment and within 30 calendar days of the date of employment.
