A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-560-A
Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB); documentation of the screening shall be submitted at the time of employment.

The documentation of a negative TB screening for Staff 2 was obtained 3 days after employment and Staff 5 was obtained 4 days after employment.
Plan of Correction: The vendor will obtain at the time of employment.
Standard 8VAC20-790-560-B
Staff shall be evaluated by a health professional to determine to be free of tuberculosis at least every two years from the date of the initial screening.

The TB screening for Staff 4 was overdue by 7 weeks.
Plan of Correction: Staff will obtain a updated screening.
Standard 8VAC20-790-580-A
The vendor shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The documentation for Child 3 was obtained over 2 weeks after the child first attended the center.
Plan of Correction: The vendor will obtain before children attend.
