Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 6/21/21 and concluded on 6/23/21. The center owner was contacted by telephone to initiate the inspection. There were 15 children present and 3 staff. The inspector emailed the center owner a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
3Evidence:
1. The record for child 1 (date of enrollment: 6/21/21) does not contain documentation of immunizations.
2. The center owner confirmed that the record for child 1 is lacking documentation of immunizations.
Evidence:
1. The most recent tuberculosis test for staff 1 is dated 1/18/19. Staff 1 was due for an updated tuberculosis test or screening in January 2021.
2. Staff 1 confirmed that her tuberculosis test is overdue.
Evidence:
1. The record for child 1 (date of enrollment: 6/21/21) contains only one name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached, where two are required. Child 1's record also does not contain documentation of viewing proof of the child's identity and age.
2. The center owner confirmed that the record for child 1 is lacking one of the two required emergency contacts and documentation that the child's birth certificate was viewed.
