During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/1/24 and completed on 10/1/24. The on-site inspection began at 11:30am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff 2 began employment at the center prior to the results of the national criminal background check being received.
Plan of Correction: The center responded with the following: Going forward, center owner will ensure that new staff will not be employed until the results of the fingerprint checks are received.

Repeat Violation
A tuberculosis (TB) screening is required every two years from the date of the last screening.

An updated TB screening for staff 3 was not completed and was overdue by four years.
Plan of Correction: The center responded with the following: Staff 3 will obtain an updated TB screening. Staff owner will ensure all staff complete TB screenings in the required timeframes.

Children's records are required to contain the work phone number and place of employment of each parent who has custody and the name, address and phone number of two designated people to call in an emergency if the parent cannot be reached.

The record for child 1, who has been enrolled for 2 years, does not contain the employment information for the custodial parent. The record for child 2, who has been enrolled for 4 months, contains only one emergency contact person.
Plan of Correction: The center responded with the following: The missing information for child 1 and child 2's records will be obtained today at parent pick-up.

Repeat Violation
Documentation of at least two references as to character, reputation, and competency are to be checked prior to employment.

Staff 1, who has been actively working at the center for 2 years, has only one completed reference verification on file.
Plan of Correction: The center responded with the following: The second reference check for staff 1 will be completed and placed in the staff's record. Going forward, center owner will ensure two reference checks are obtained for all new staff.
