Inspector Notes
An unannounced monitoring inspection was conducted at 1/23/23. The provider and assistant were present with 6 children, a total of 16 points. The children were observed playing and eating lunch. Lots of interaction between children and caregivers. The provider has lots of space and lots of materials for the children to explore. Today's lunch was Chicken, rice and veggies.
A violation notice was given to the provider along with this inspection summary.

2Evidence: The providers and assistants sworn disclosure were signed on 3/8/2017 and are over 5 years old. The central records checks expired 3/28/2017.
Evidence: Based on the review of 4 children's records, child 1.2.3 and 4 are missing physicals. All children have attended the daycare for more than 30 days.
