During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was completed on 05/22/2025, as a part of the licensure period. The on-site inspection began at 11:40 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 13. The inspector reviewed 2 children?s records and 2 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/29/2025. A POC submitted after this date will not appear on the public website.
Violations
6The provider had the Supplemental page from the inspection dated 1/13/2025 posted conspicuously.
Child #2 was seen sleeping in a bed, unattended by an adult.
Child #2, who is 7 months old, was seen sleeping with a blanket wrapped around them.
Child #2 did not have written authorization for parents for the caregiver to provide emergency medical care.
A tube of diaper cream laying on a bed that an infant was laying on.
Child #1 and Child #2 did not have documentation of the verification method of the child's age and identity.
