Inspector Notes

An unannounced Subsidy Inspection was conducted on 11/9/2022. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff/caregiver record review, the Licensee failed to ensure that all care givers have current First Aid and CPR

EVIDENCE:
1. Record for Staff #3 did not include documentation to confirm current First Aid & CPR certification. The record shows that CPR expired 2/28/2019 and there is no FA documentation in the record.
Plan of Correction: Staff will complete First Aid and CPR certification training. Documentation of the training will be kept in the staff record.
