Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated and concluded on 3/10/2021. The director was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the director a list of items required required to complete the inspection. The inspector reviewed 1 child record, 1 staff record, 2 household records submitted by the facility to ensure documentation was complete.

The information gathered during the inspection determine compliance there were violations with applicable standards or law.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Standard 22VAC40-111-180-A
Based on staff records review, the provider did not maintain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1 Tuberculosis Screening expired on April 18, 2021.
2. Household member #1 Tuberculosis Screening expired on April 18. 2020.
3. Household member #2 Tuberculosis Screening expired on May 7, 2020.
Plan of Correction: Tuberculosis Screening completed and it's valid next two years.
