Inspector Notes

A complaint investigation was initiated on 3/1/2023 and concluded on 3/28/2023. The complaint consisted of allegations received by the Northern Licensing Office on 3/1/2023 regarding supervision and infant care. Based on staff interviews, review of attendance and staffing sheets for the Infant/Toddler classroom and review of incident reports, a preponderance of evidence supports the allegations regarding supervision and infant care. Therefore, the complaint is determined to be valid. Areas of non-compliance are identified in the Violation Notice.

Based on record review and staff interviews, the center did not ensure that the required ratio of 1 staff to 4 children for children ages birth to 16 months was maintained.

Evidence: On 2/27/2023 from approximately 7:55am to 8:00am Staff #1 was present with approximately five to seven children between the ages of six months and two years old in the Infant/Toddler Classroom.

Based on staff interviews, the center did not ensure that infants were protected from older children.

Evidence: On 2/27/2023 Staff #1 and Staff #2 were present with approximately five to seven children ages 6 months to 2 years old in the Infant/Toddler Classroom. At approximately 7:55am, Staff #2 left the room to assist in the front office. Staff #1 took Child #3 into the infant side of the room to complete a diaper change, leaving the gate open which separates the infant and toddler play spaces. While Staff #1 was completing Child #3?s diaper change, Child #2 (Age 2) went to the infant area and scratched Child #1?s (Age 6 Months) face leaving approximately 16 scratches on the left side of the child?s face, breaking the skin in several areas.
