During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 06/12/25 and completed on 06/30/25, as a part of the licensure period. The on-site inspection began at 9:30am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 12 staff. The inspector reviewed seven children's records and seven staff records on-site. Additional documentation was reviewed electronically on 06/30/25. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
(Repeat Violation) Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening.

The most recent TB in the record of Staff #2 was dated more than one month after their first day of employment. A previous TB in the file expired four months prior to the first day of employment.
Plan of Correction: Updated TB already obtained. In the future, the center will ensure returning staff have obtained a TB within the required timeframe.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the Twos classroom, cleaning materials were observed in an unlocked cabinet in the bathroom.
Plan of Correction: The cabinet will be fixed so staff can store the cleaning materials in a locked place as required.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

During the inspection on 06/12/25, there were 16 children present in the twos classroom, but only 11 children were singed into the written record of daily attendance for the group. In the preschool classroom, 15 children were present, but only 14 were singed into the written record of daily attendance for the group.
Plan of Correction: Corrected on site. Staff added the missing child to the daily attendance in the preschool classroom. Staff were reminded to add children as they transition from other classrooms into their own as the transitions occur.
