Inspector Notes
An unannounced inspection was initiated on 06/12/2025 and completed on 07/08/2025 in response to a complaint received by the licensing office on 05/29/2025 relating to allegations of the physical plant, staff qualifications and training, and supervision. The inspector reviewed two staff records on-site on 06/12/2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
