Inspector Notes

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A non-mandated monitoring inspection, resulting from a complaint, was initiated on 06/14/2021 and was completed on 06/17/2021. The Licensing Inspector conducted telephone interviews with 2 staff members on 06/14/2021 and 06/16/2021. The inspector reviewed 1 staff record during a virtual inspection on 06/16/2021 to ensure documentation was complete. Information gathered during the inspection determined that the complaint was not valid. Violations with the applicable standards or law were cited but are not related to the complaint received. The violations cited are documented on the violation notice issued to the facility.

Standard 22VAC40-185-240-A
Based on record review and discussion with the staff, the facility failed to ensure that staff shall receive all trainings required by the standards by the end of their first day of assuming job responsibilities.

Evidence: The record for Staff #1 contained a documented hire date of 06/02/2021. Staff #2 stated that Staff #1 assumed job responsibilities on 06/02/2021 and was not provided the trainings as required by the standards.
Plan of Correction: Make sure all are trained on job responsibilities, do the required trainings and have orientation before allowed on the job site.
Standard 22VAC40-185-240-B
Based record review and discussion with the staff, the facility failed to ensure that by the end of the first day of supervising children, staff shall be provided in writing with the required information as stated in the standards.

Evidence: The record for Staff #1 contained a documented hire date of 06/02/2021. Staff #2 confirmed that Staff #1's first day supervising children was 06/02/2021 and that Staff #1 was not provided in writing with the policies/procedures/information as required by the standards.
Plan of Correction: Make sure to go over policies and procedures, give them a copy of an employee handbook and parent handbook and have staff sign off that they receive it prior to starting employment.

Based on record review and discussion with the staff, the facility failed to ensure that staff records contain all of the elements as required by the standards.

Evidence: The record for Staff #1 contained a documented hire date of 06/02/2021. Staff #2 stated that Staff #1's first day assuming job responsibilities was 06/02/2021.The record for Staff #1 did not contain written information to demonstrate that the individual possesses the orientation training required by the job position.
Plan of Correction: Will make sure that all staff and volunteers have orientation before being around the children.
