Inspector Notes
An unannounced monitoring inspection was conducted on 07/06/2022. There were 29 children, ages 5 months-12 years, and 7 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in the following: having breakfast, in free choice activities, in story time and during outside large motor play. A total of 5 children?s records and 5 staff records were reviewed. The inspector discussed the following with staff: outside play, record keeping and training. The inspector arrived for the inspection at 9:10 am and departed at 12:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
4Evidence:
1. The record for Staff 4 contained a documented hire date of 03/01/2022. The central registry search was dated 04/18/2022.
2. The record for Staff 5 contained a documented hire date of 02/14/2022. The central registry search was dated 04/10/2022.
Evidence: The record for Staff 1 contained a documented hire date of 06/20/2022. The tuberculosis screening was dated 05/11/2022; the tuberculosis screening had not been completed within the last 30 calendar days of the date of employment.
Evidence: The children in the Pre-K/School-Age Class were observed immediately returning to activities upon the completion of their breakfast. The children in the Pre-K/School-Age Class did not wash their hands with soap and running water or disposable wipes after eating a meal.
Evidence: There were two bottles of milk/formula in the infant room that were not dated or labeled in a way that identified the owner. The staff verified that the bottles were brought in from home.
