During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on 03/09/2026 and completed on 03/12/2026 in response to a self-report received by the licensing office on 03/03/2026 relating to programs. The inspector reviewed 1 child?s record and 1 staff record on-site.

This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/19/2026. A POC submitted after this date will not appear on the public website.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

As Class 1 walked to the playground on 02/25/2026, Child 1 (age 3 years) broke out of the line, passed the other children, and moved to the front of the line. Staff 1 tapped Child 1 on the forehead twice, hard enough to make the child?s head move backwards, and sent the child to the back of the line.
