Inspector Notes

A monitoring inspection was initiated on 11/ 29/2022 and concluded on 11/29/2022. There were 56 children present, ranging in ages from 18 months to 6 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 1 medication authorization and 5 staff records were reviewed. Information gathered during the inspection determined noncompliance with applicable standards or law and violations
were documented on the violation notice issued to the facility.

Based on record review, the center failed to obtain all out-of-state background checks.

Evidence:
1. Staff #2's sworn disclosure statement dated 03/31/2022 stated they had lived in two other states outside of Virginia within the last five years.
2. An out-of-state child abuse and neglect check was required to be sent out within 30 days of employment. There was not an out-of-state child abuse and neglect check completed for Staff #2. Date of hire for Staff #2 was 04/01/2022. An out-of-state child abuse and neglect check was required to be sent out by 05/01/2022.
3. Administration verified the date of employment for Staff #2 and that there was not an out-of-state child abuse and neglect check completed.
Plan of Correction: Administration will get the out-of-state background checks completed within the correct time frame.
