During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on April 21, 2025 and completed on June 9, 2025 with a local agency, in response to a complaint received by the licensing office on April 21, 2025 relating to allegations of forbidden actions. The inspector reviewed 3 children?s records and 3 staff records on-site on April 21, 2025 and June 3, 2025. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 10, 2025. A POC submitted after this date will not appear on the public website.
Violations
2On April 14, 2025 in Classroom #1, Staff #1 approached Child #2 (age 3) by the changing area, grabbed their right forearm and pulled the child towards them, causing the child to fall to the ground on their left knee. Staff #1 forcibly pulled off the child?s jacket. Staff #1 then turned to the row of children behind them, pointed a finger in Child #3?s (age 4) face and yanked Child #3?s left shirt sleeve to forcibly remove it. Staff #1 then walked behind Child #1 (age 4) who was sitting in a chair, grabbed them by their upper arms to lift them and turned Child #1 around towards them, then pushed the child away in another direction, causing the child to open their mouth to appear like they were screaming and stomp away to another area of the classroom where it continued.
On April 14, 2025, after nap time, Staff #1 grabbed Child #1 (age 4) underneath their arms and lifted them off their cot. Staff #1 pulled the child with both arms while on their tip toes across the room to a changing area where Staff #1 then forced the child to sit down in a chair.
