Inspector Notes

A renewal inspection was conducted on 08/08/2023. There were a total of 46 children with 14 staff members present. Children were observed during outside water play, tummy time, and engaged in various activities with staff members. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. During the monitoring inspection the inspector investigated a self reported allegation regarding forbidden action. The investigation determined no violations regarding the self reported allegation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of staff records, the director did not ensure that all staff completed the orientation training in subsection C no later than seven days of he date of assuming job responsibilities.
Evidence:
Staff #2 (date of hire 11/14/2022) and Staff #4 (date of hire 6/23/2021) did not have documentation of the VA orientation training on file during the inspection.
Plan of Correction: Staff #2 completed orientation training, certificates were verified. The orientation training document was not in Staff #2 file.
Staff #4 has ended her tenure with The Merit School of Arlington; effective 8/11/2023.

Based on observation, the director did not ensure that prepared infant formula was dated and labeled with the child's name.
Evidence
1 of 3 sets of bottles did not have a name and date on the bottle.
Plan of Correction: Teacher immediately corrected; Infant Teacher labeled the bottle with date and child's name.
